Lamellar endothelial transplantation (Descemet’s Stripping Automated Endothelial Keratoplasty, DSAEK)

In lamellar endothelial transplantation, only a thin posterior corneal stromal lamella is transplanted together with the Descemet membrane and the endothelium. Such an operation is indicated if there is a disease exclusively of the corneal endothelium, so that the cornea is swollen and therefore no longer sufficiently transparent due to corneal endothelial dysfunction.

The process of donor trephination and recipient corneal preparation as well as the actual transplantation, i.e. the insertion of the donor corneal disc into the recipient eye and tamponade with sterile air, is illustrated schematically and pictorially below using a specific case.
