Request for patient documentation / report

As a patient of the USZ, you (or your legal representative) can request your patient documentation online.

Third parties such as relatives are only entitled to inspect patient documentation if they can present the corresponding consent of the patient or the legal representative or on the basis of an explicit legal basis. Submit the relevant enclosures from the outset (e.g. patient’s release from medical confidentiality, living will, etc.).

This requires a release from the Health Directorate of the Canton of Zurich. The protection of patient confidentiality survives the death of a patient (cf. Information sheet of the Zurich Health Directorate regarding Access to patient documentation of deceased persons). The right to patient confidentiality cannot be inherited and does not pass to relatives upon death.

Relatives therefore have no right to inspect the patient documentation of the deceased. The Zurich Health Directorate is examining on behalf of the deceased whether their doctors can be released from their duty of medical confidentiality. For this purpose, the person who wishes to inspect the patient documentation must demonstrate a corresponding interest and comprehensible reasons.
